WeCare at The Pearl is actively seeking a motivated Social Work Assistant for our community in Rochester, NY. The Social Work Assistant provides assistance to the social work team in delivering comprehensive social services to residents and their families in a nursing home setting. The assistant supports the social work team in assessing residents' psychosocial needs, developing care plans, and coordinating services to meet those needs. The assistant also provides emotional support and counseling to residents and families, facilitates support groups, and assists with discharge planning and community resource referrals.

· Reviews and follows up on any concerns for the residents and staff with all incident reports.

· Works with caregivers of the nursing unit and all departments to provide the residents with a fulfilling living experience.

· Keeps supervisor informed of unusual occurrences that relate to the health and safety of residents.

· Oversees care plans, MDS, and quality improvement processes.

· Keeps Admin fully informed of stats, progress and achievements.

· Must have the ability to deal tactfully with personnel, residents, family members, visitors and the general public.

· Must be able and willing to work well with inter-disciplinary teams.

· Any other duties as assigned
